PAGCOR Self-Exclusion: How to Apply Step by Step
Want to apply for PAGCOR self-exclusion? Choose 6 months, 1 year or 5 years, prepare your ID and photos, and send the form. Here's each step.
Self-exclusion bars you from all PAGCOR-operated and regulated gaming sites and establishments for the period you choose. It's a strong step, and taking it is a good decision. For signs of problem gambling and other support, see our responsible gambling page. This guide focuses only on how to apply.
Before you start:
- Decide on a period: 6 months, 1 year or 5 years.
- Have a valid government-issued photo ID with your signature and birthdate.
- Get two 2x2 colored photos taken in the last 6 months.
What self-exclusion does
- According to PAGCOR, the exclusion is enforced at all PAGCOR-operated and regulated gaming facilities, wherever you applied.
- Your name goes into PAGCOR's National Database of Restricted Persons (NDRP), and PAGCOR shares your particulars with gaming establishments so they can stop you from playing.
- The form says you can't collect winnings or recover losses from gaming while the exclusion is in effect.
- When the period ends, the order is lifted automatically.
Choose your period
| Period | Can I cancel early? | What happens at the end |
|---|---|---|
| 6 months | No. The first 6 months can't be revoked. | Lifted automatically. |
| 1 year | Not in the first 6 months. After that, PAGCOR has a Petition to Amend the Exclusion Period form. | Lifted automatically. |
| 5 years | Not in the first 6 months. After that, the same petition form. | Lifted automatically. |

You can re-apply when it ends by ticking "Re-application" on the form.
Step 1: Download the form
Get the Self Exclusion Application Form from PAGCOR's player exclusion page. Only use the form from PAGCOR's own site.
Step 2: Fill it in
- Write in block letters: full name, ID presented and ID number, date of birth, contact details and address.
- Tick the type of gaming you visit regularly (for example e-Games, casino or sports betting).
- Choose your period and sign the terms.
Step 3: Prepare the requirements
- Photocopy of a government-issued photo ID with signature and birthdate
- Two 2x2 colored photos: clear, taken within the last 6 months, full face toward the camera, no hat, glasses or face mask
PAGCOR notes that incomplete applications may not be processed.

Step 4: Submit
- By email: PAGCOR's exclusion page lists ResponsibleGaming@pagcor.ph. If you don't submit in person, PAGCOR says a selfie or video call is required to confirm your identity.
- By envelope: PAGCOR's exclusion forms page asks for the signed form and requirements in a sealed long brown envelope marked "APPLICATION FOR EXCLUSION ORDER", Gaming Licensing and Development Department, sent by personal delivery or courier to the PAGCOR corporate office address shown on that page.
Check PAGCOR's page for the current address before you send, as office details can change.
Step 5: After you apply
- Close or self-limit your online casino accounts too, and remove gambling apps.
- If a casino still lets you play, tell its support and keep a record.
- Tell someone you trust. Our responsible gambling page lists support options.

For a family member
A spouse, a child aged 18 or over, or a parent can apply for Family Exclusion, for 6 months, 1 year or 3 years. It needs a different form and extra documents, such as a birth or marriage certificate. See PAGCOR's exclusion page.
Red flags
- Anyone charging a "processing fee" to file self-exclusion for you. The form comes from PAGCOR's website.
- Forms or links sent by strangers instead of PAGCOR's own site.
- A site that offers to "remove" you from the NDRP early.
See gambling anti-fraud for more warning signs.

FAQ
Does self-exclusion cover online casinos?
PAGCOR says the order covers all gaming establishments and sites it operates, licenses or regulates. Unlicensed sites aren't bound by it, which is another reason to avoid them.
What if my casino account still has money in it?
Ask the casino how your balance will be handled before or as you apply. Our account suspended guide covers what to ask.
Can I change my mind?
Not during the first 6 months. After that, PAGCOR has a petition form to amend the period.
Does it cost anything?
PAGCOR's exclusion page doesn't list a fee. Be wary of anyone who asks you to pay to file it.
